The Grange Reablement centre provides short-term intensive therapy and reablement for medically optimised patients before returning to their usual place of residence.
It will focus on rehabilitation, planning for patients’ discharge, and addressing their ongoing care and therapy needs.
In a bid to promote patient independence and enhance functional skills, we will be implementing an innovative model of care, including the development of reablement goals and the use of a therapy room equipped with specialist machinery. Our dedicated dining area will encourage patient independence, and we will promote the self-administration of medication.
