Here at The Grange Reablement Centre, every member of our team welcomes you with a warm smile. Alongside the compassionate support workers and attentive manager, we work with great effort to help residents settle comfortably into their recovery journey. We create a supportive, caring environment that promotes peace of mind and empowers each individual to flourish and regain independence. The care and companionship shared between staff and residents foster a sense of community that enhances the healing process.

Our highly skilled and experienced support workers provide round-the-clock care tailored to meet the unique needs of each resident. Under the dedicated supervision of the registered manager, we deliver care that encompasses physical, social, psychological, and environmental well-being, ensuring a holistic approach to recovery.

The staff at The Grange are extensively trained and committed to delivering compassionate, person-centered care. Senior support workers are qualified to administer prescribed medications, ensuring that medical needs are managed seamlessly. We have access to on-call doctors, district nurses, and Emergency Care Practitioners (ECPs) who can provide prompt medical attention, prescribe medication, or arrange hospital admission if required.

Understanding that no two people are the same, we take the time to get to know each resident personally from the very beginning. This allows us to create individualised care plans that provide the best possible experience and outcomes.

At The Grange , we believe that recovery is about more than just regaining strength—it’s about rediscovering a sense of purpose and joy. Through the support of our dedicated staff, the companionship of fellow residents, and the engagement of family and loved ones, we replace the anxiety of recovery with renewed confidence and hope for the future.

 

Who Will I See

 

A team consisting of physiotherapists, occupational therapists, reablement assistants, nurses and pharmacists with GP support will be available to provide assessment and goal planning in collaboration with you.

What Can I Expect?

Alongside the compassionate support workers and attentive manager, we work with great effort to help residents settle comfortably into their recovery journey. We create a supportive, caring environment that promotes peace of mind and empowers each individual to flourish and regain independence. The care and companionship shared between staff and residents foster a sense of community that enhances the healing process.

You will have a therapy session 1-2 times a day for exercise, mobility and individual goal practice plus other activities. In order to achieve your goals and best possible outcome it is expected that you willingly participate in every session with motivation.

We expect your length of stay to last no longer than 14 days.

Discharge planning is part of your rehabilitation plan and will commence from initial assessment.

If there are any outstanding goals not achieved whilst with us, these can be referred on to the necessary community teams that are able to assess and continue goal/rehabilitation progress once home.

A referral to social services for a care package may be required if there are any ongoing care needs.

An access visit to your home by an occupational therapist or assistant practitioner may need to be completed. The access visit is to assess your home environment for falls risks, equipment and adaptation needs to promote independence and enable a safe discharge home.

To support your stay at The Grange, the following items will be required:
• Day and night clothes and appropriate footwear for mobility
• Toiletries
• Mobility aid if you have been provided with one.

 

Visiting hours

 

We encourage friends/family members to visit while you are staying at The Grange. Please supervise children at all. We do not have a designated visiting times but we do ask that you avoid meal times.

Visitors will be encouraged to continue patient care activities where safe and appropriate.

Smoking

The Grange has a no smoking policy on all its sites in accordance with government legislation. Smoking is not permitted on The Grange premises or grounds, except for patients only. There is an allocated smoking shelter at the back of the building located in the garden area, but this is strictly for patients only.
